Why would I need endodontic surgery?

Endodontic surgery is performed when traditional root canal treatment does not achieve the expected results and is the only alternative to having the tooth extracted.

Indications for surgery include:

  • Complications resulting from the anatomical structure of the tooth not allowing proper cleaning or closing of the canal. i.e. extreme curvature or calcified root
  • Prosthodontic restorations causing difficulty in conducting root canal treatment. i.e. prosthetic crowns or crown inlays block access to the canals and removal may cause substantial damage to the entire tooth.
  • A horizontal fracture of the root or decay in the vicinity of the root-end.
  • Material that cannot be removed under microscopic nonsurgical root canal treatment. i.e. broken metal files
  • Large areas of inflammation disqualifying nonsurgical root canal treatment.

Although there are many surgical procedures that can be performed to save a tooth, the most common is called apicoectomy or root-end resection. When inflammation or infection persists in the bony area around the end of your tooth after a root canal procedure, our surgeon may have to perform a root-end resection.
